Use Google Docs to Create a Form

Want a cheap and easy way to create a contact form Online?

The first step would be to login to docs.google.com.  From the “New” menu, select “Form”.  This will start your new contact form that your visitors can use.

Next, you can customize your form to meet your own needs.  To add something to your form click on the “Add a Question” button at the top.

Some of the values you can add to your form include:

  • Text
  • Multiple Choice
  • Check Boxes
  • Choose from a List
  • Paragraph of Text
  • Scale (1-n)

To setup a basic contact form, we can add and edit two Text boxes (one for name and the other for e-mail address) and then one paragraph of text for their message.  You can edit the choices by clicking the pencil icon, and delete them using the trash icon.

Once you have the form the way you want it, click on “More Actions” at the top of the screen and then “Embed”.  This will give you your embed code you can copy from here and paste into your web site.  Now when somebody inputs their information into your form, you can see the results in a Google Docs spreadsheet.

I have also used this myself, so you can see an example of that here.  It should be selectable now, showing all of your results with a column per topic.  This makes an easy and cheap way to setup a contact form, a feedback and suggestions page or anything else you can use your imagination to create.
